I purchased this form charger three years ago. It worked perfectly for 3 years. It recently quit charging properly. I contacted FORM customer service and they were immediately helpful and responsive. They assisted me with troubleshooting to figure out what was going on. When we determined that it was definitely the charger that wasn’t working they completely replaced the charger free of charge for me under their warranty program. They mailed the new charger out quickly and it was at my house in 4-5 days. If you purchase a charger I would recommend registering it with FORM in case you need to file a warranty issue. I highly recommend this charger and will continue supporting this brand! ... show more

I was looking for another charging, having used the "stock" charger that comes with club cars. The stock charger was big, clunky, an ugly tin can that frankly seemed to be causing some issues with the batteries. It was also very old... Enter the FORM 15...this thing is AWESOME. Super small, super light, not awful looking, simple indicator light...and most importantly, charges without issue! It comes with a mounting kit so no more chunk of metal on the garage floor...now, the charger lives on the wall with the chord rolled up under it. The other thing I'll mention: when I left the old stock charger plugged in beyond full charge, I'd get the dreaded "rotten egg" smell coming from the batteries. With this charger, no such issue...I've left it plugged in all day, and it's clearly more intelligent and cuts off/trickles the charge appropriately. ... show more

My batteries were old so I replaced these when my charger stopped working. That didn't work so I bought this charger and It too failed to charge my new batteries. I contacted their customer service through their WEB site and filled out the forms, answered the trouble shooting form questions and my computer didn't access the last "Schedule a Call back" time step. So I sent an email to their Customer Service with all the pertinent information on the issue and the symptoms etc. Ron, their Senior Product Specialist, called me and explained a couple of potential issues with solutions for each. He promised to and immediately followed up with, an e-mail that detailed our conversation and it included each potential isse and the relevant solutions. I ruled out two with his diagnostic recommendations and used his third solution which took me realtime 15 minutes to complete and everything is working perfectly now. I am amazed at how knowledgeable Ron is/was, how quickly he was able to ID the potential issues, AND most importantly how clear and easy to follow his solution was. Each step clearly identified, even with potential issues ID'd to avoid as I proceeded, and all in what I call "lay terminology" with diagrams and pictures that were easy to understand. What a pleasure to work with a really knowledgeable and patient (I asked a lot of questions and it was obvious he wasn't working from a script as he answered directly the specific questions I asked.) customer service representative/department. I highly recommend this company and product, which is working flawlessly after a month of use. Amazing and Thanks Greg Note the attached video is showing the symptoms the charger exhibited while not working, which was due to my carts computer issue, NOT the chargers issue. :-) ... show more

Edit 4/25/23 Changing my review to 5 stars! If I'm wrong, I admit it. The error was in fact MINE. The charger worked and "didn't work" exactly as it was supposed to. I "assumed" my water level was correct in all my batteries and it wasn't. Ron, from customer service contacted me, ran through some questions and things to check. Based on their info, when I got home that night, sure enough, it was the batteries that were the culprit. After filling them to the proper level with distilled water, the charger went right to it and worked flawlessly. A++ for a product thats smarter than I am and A+++ for superb customer service from Form and Ron! He even called me back after a few days to verify everything was good!! Who does that! Highly recommend this excellent smart charger for the functionality itself as well as excellent customer service that stand behind their product. Well, I bought the charger this Jan 2023 and I was raving about it. It charged my 01 CC DS without issue, it was quiet, and it looked nice mounted on the wall in my garage. 4 months later, its a paperweight. The fan kicks on and off and the light just flashes red and green. It no longer charges my cart. Hopefully its warrantied because at $300 its a heck of a hit to the wallet to have to purchase another one already. ... show more

    • Great option for a bad ERIC charger, with Caveats

At last count, there are about 16 importers selling this same exact (internal) charger with in some cases (as this one) proprietary external housings. Sorry, but it is what it is. Do they all work, YES. Are they as fast as the OEM ERIC, NO. Are they about 60-70% the cost, YES. Bottom line, With these chargers, the entire REAL DEAL is longevity, which of course, I cannot speak to after load-testing to a peak of 13.1A (which is actually better than the "MODZ" version of the same internals) - I can say that the thicker extrusion RE the body of the charger may be a better heatsink than some others that use the same China Boards and Components - and this one does run cooler to the touch than the lighter ones. THAT said, the fan becomes less and less help as the body of the charger becomes thicker and less condusive to heat-sinking which results in FAR higher internal temps - so as time and use unfolds, the entire situation may change, and i will update the review. For now, YEP. Another China Charger. Price Point is GREAT and to be honest, the wire gauge and connectors are of far better quality than the MODZ so I am optomistic. I would wholeheartedly recommend any of these China Chargers at the price point if You are willing to roll the dice and don't want to replace your OEM at 2-3X the outlay, with Dealer Support and proven reliability. The lighter, fan-cooled versions of these (sold under several brand names, such as MODZ, Alitree, etc) share the internals with this one, and as stated, a heavier housing may or may not result in a longer lifespan in normal use. I would err on the side of caution though and go with this unit. ... show more
