It has an original pad inside the pillowcase and then two extra pads in case you wanted it to be firmer. So it is adjustable. I'd recommend it. It doesn't slide in bed, stays in place. It fits my back, others were too big or too high. Although I ordered the extra inside pillow, I didn't really use it; the hight of the pillow itself is more than enough to relief the pain and pressure. I used it for a few days when my back was really hurting then I stopped using it every day. I felt that it got my lower back in place. I use it now as needed. I started having back pain since I got a new fancy mattress that is slightly too soft than what I’m used to. It was too late to return it so I decided to figure out a way to make it work. I added a firming topper and then researched into lumbar support options for sleeping and came across this little pillow. Before I bought a different lumbar pillow and that one proved itself useful only for sitting. This pillow however is comfy to sleep with. Now, I won’t say I don’t feel it (before back pain sleeping without any lumbar support was better) but this one enables me to wake up pain free and not have a stiff back. I took out the second layer so it’s at its flattest and I’m happy to report that I can finally wake up in the morning and not resume my search of back pain relief. This pillow is great! It doesn’t get hot, memory foam contours and supports, it is puffier on the sides which provides additional back support while sleeping on my side and it’s not bulky like other lumbar pillow I got so it doesn’t bother my partner. ... show more

Was so excited to get this and disappointed. I needed a pillow to place under my waist to sleep on my side due to pain in hips and leg as recommended by my doctor well this ain’t it. Way to flat, I can continue to use my throw that works way better than this. Highly NOT recommended

Love this pillow. I first bought it to use under my lower back when sleeping which kind of works for a while but then gets a little uncomfortable after a few hours. You also have to lay still and keep it in the same position all night for it to work and it might still get uncomfortable after a while. Now on occasion I'll just take out the small pillow inside and use that for my lower back at night which works better for me. I started using the pillow for my lower back on the couch instead and liked it a lot. I also bought another one for my office chair. It works great for long car rides too since it's very soft and comfortable and the shape of it gives you good lumbar support. ... show more
