It's adjustable to fit little heads with an easy-to-turn crank and chin strap. They both felt more comfortable riding their bikes (and so did Grandma). This helmet must be comfortable because my daughter has refused to wear her helmets in the past but has no issue putting this one on every time she rides her bike. My daughter is 4 (almost 5) and wears the helmet at just about the smallest setting. It fits her head well and doesn't wobble around on her head. It is easy to adjust the size with the dial on the back.
