I bought this item for my 2 month old (12lbs) at the time. It’s an apron attached to a skirt, I put her own long sleeve on and it blended well. I kept seeing the reviews complaining about how it ran small, and it certainly does. The 3-6 month size is max for a 3 month old. My baby had a tiny gap on the skirt but it was unnoticeable! Worked out great for me, as it was extremely difficult to find 0-3 month costumes. DEFINITELY go up a size or two if you have a chunk. Man the shipping was a little long but it was so well worth the wait and the price. My then 21 month old little girl was so cute in this on Halloween- it came with a little hood that you could take on and off- its separate from the costume and the main dress is like a little bodice and the the back is out so I put on a black long sleeve shirt underneath with black leggings and it was still cute.
