You may download the name tag template (MS Word File) from the manufacturer's website, and easy to use. Also, very sturdy with high adhesion so will stay longer as you desire. No residual and easy to remove.

I highly recommend Avery Flexible Name Tag Stickers. I used these to create name badges for a banquet with 100 people at it by setting up an Excel spreadsheet with the attendee's name, club name, and the award they were being presented with. I then did a "mail merge" to put the information on the badges, along with the logo of the sponsoring group we all belong to. The badges were individualized and looked great! Attendees appreciated that they didn't fall off their shirts during the event and no glue residue remained on their apparel when they did remove them. Best quality name tag I ever used. Easy to use online templates, super durable compared to the everyday paper name tag. The don't peal up at the corners as bad as most. They stay on and in place yet peal off easy when your done.
