I bought a medium. It was a little loose in the chest as I am not very busty (32DD) but comfortable. I opted to wrap it around myself instead of the very open deep V. My boyfriend loved it. I wore nipple covers and no bra.

Due to this, my boobs are bigger and when they're natural and bigger, they're gonna sag a bit. My point being, wear a bra with this. (I'm rarely wearing a bra because we really don't need to!
